    Any civil rights complaints regarding the Food Service Program should be directed to Mr. Peter E. Genovese III, RSBO,QPA, School Business Administrator/Board Secretary at 540 Broadway, Long Branch, NJ, 07740 or 732-571-2868 extension 40100.  All complaints will be reveiewed thoroughly for  resolution.  If a resolution cannot be reached, the complainant will be advised to contact the U.S. Department of Agriculture by following these prcocedures:
     

    If you wish to file a Civil Rights program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, found online at http://www.ascr.usda.gov/complaint_filing_cust.html, or at any USDA office, or call (866) 632-9992 to request the form. You may also write a letter containing all of the information requested in the form.
